Immunization Requirements

All students must remain up- to- date with state required immunizations, per 4 AAC 06.055.

Students who are out of compliance according to the Alaska State guidelines may be excluded from school.

All students enrolled in Alaska schools must be immunized against:

• Diphtheria, Tetanus, Pertussis (DTap/DT/Td/Tdap)
• Polio
• Measles, Mumps, Rubella (MMR)
• Hepatitis A
• Hepatitis B
• Varicella through 6th grade

The number of doses and type of vaccine required depends on your child’s age, grade and prior vaccination history. Check with your health care provider for details.

Exemptions

Complete and submit the following forms if you are seeking an exemption for your child.

• Medical Exemption -A physician's statement is required if there are medical reasons a child cannot be vaccinated.
• Child in Transition - Federal law allows for a 30 day provision.
• Religious Exemption - Must be notarized and submitted each school year.
