Registration

New to District Students: Required to provide a health history form and immunization records to the health office. Returning Students: Submit updated immunization records and any new health concerns to the health office.

Health Concerns

Parents/guardians of students with health concerns should contact the health office to develop a plan for nursing services. Working together with you and your doctor, our health team will provide frequent services for:

• Daily medication needs
• Allergies/Anaphylaxis
• Asthma
• Diabetes
• Seizures
• Tube feedings
